CSD Harassment Intimidation and Bullying (HIB) Form
The Centralia School District is committed to providing a safe and civil environment for learning. Such an environment is free from harassment, intimidation, or bullying. Our district defines harassment, intimidation, and bullying as any intentionally written message or image - including those that are electronically transmitted - or a verbal or physical act (including but not limited to those motivated by race, color, religion, ancestry, national origin, gender, sexual orientation, mental or physical disability, or other distinguishing characteristics)

TITLE IX
Title IX is a federal civil rights law that prohibits discrimination based on sex in any educational program or activity that receives federal financial assistance. This law protects faculty, staff, and students from various forms of discrimination, including sexual harassment, dating violence, domestic violence, stalking, gender identity issues, and pregnancy discrimination. Additionally, Title IX prohibits retaliation against anyone who files a complaint or participates in an investigation.
The Centralia School District is dedicated to providing a safe learning environment for both students and staff. We encourage the prompt reporting of any incidents. Reports can be submitted through our online report form or by contacting the Title IX Coordinator, Samantha Mitchell, at (360)330-7600. Anonymous reports are also accepted.

What happens after I submit a report?
Both the Harassment, Intimidation and Bullying and the Sexual Harassment reporting forms are submitted the HIB and Title IX Coordinators. While each situation is unique, the following generally occurs once a report is received:
- The form is sent directly to the HIB or Title IX coordinator in the School Director's office.
- Parent or guardian of the student and the school principal are notified that a concern was reported.
- The principal will ensure a temporary plan is in place to keep the student safe and supported at school.
- The coordinator ensures that the reported concern is investigated.
- At the conclusion of the investigation, next steps for action, if needed, are determined and corrective measures are put in place. The coordinator works in partnership with the school as well as Human Resources and/or other necessary departments.
- Parent or guardian of the student are notified of the results of the investigation and the process to file for an appeal.

CSD also provides equal access to the Boy Scouts of America and other designated youth groups.
If you believe that someone has experienced discrimination or discriminatory harassment at school in the work environment, you have the right to file a formal complaint.
