On Monday, August 18th from 10am-1:30pm, Schine Health will be offering FREE sports physicals for all 6-12th grade student-athletes!
Parent attendance is STRONGLY ENCOURAGED. Please arrive with the completed forms SIGNED BY PARENT/LEGAL GUARDIAN, to be granted a Sports Physical. There will be forms available to fill out on site. Please print the below required forms for your child's sports physical. Also, please bring your insurance card if you have one! (Individuals will not be billed)
