IMPORTANT: Transportation Requests Require School Registration

Students must be fully registered in Skyward for Centralia School District before a transportation request can be submitted.

This transportation request form does not constitute or replace new student registration. Completing this form does not enroll a student in Centralia School District and does not create eligibility for bus transportation.

New students must first complete the district’s enrollment process and be officially registered in Skyward and with their school. Once registration is complete, a transportation request may be submitted.

If your student is not yet registered with the district and their school, please complete the enrollment process before submitting this form. This form will not be processed until that happens.
